Why Fill Them Out Ahead of Time

When your forms are in before you arrive, Dr. Kaceli can review your history before she meets you. Check-in goes faster, and more of your appointment goes to the exam and the conversation about your goals.

It also gives you time to look up things that are hard to remember on the spot, like medication names or the date of your last cleaning.

What the Forms Cover

Most people finish in a few minutes. You'll be asked for:

  • Your medical history, including conditions, allergies and medications
  • Your dental history and any current concerns
  • Contact details
  • Insurance information, if you have dental coverage

Have your insurance card nearby before you start. You'll still want to bring it, along with a photo ID, to your first visit.

Adult and Child Forms

Choose the form that matches your office below. Where a separate child form is offered, a parent or guardian should fill it out for their child. If you don't see a child form for your office, call and the team will tell you what to fill out.

Your Privacy

The forms are submitted securely online, and the information you enter goes into your patient record. Our privacy policy explains how website information is handled. If you'd rather not enter something online, leave it blank and talk to the team at your visit.

Prefer Paper?

That's fine. Call the office and let them know you'd rather complete the forms when you arrive, then come in a little earlier than your appointment time. You can find both phone numbers on our contact page.

What if I make a mistake or my information changes?
Let the front desk know at check-in or call the office. They can update your record.
I'm already a patient at one office. Do I need new forms for the other?
